About the Role

Care Managers are expected to maintain resident safety, dignity, and confidentiality while documenting care in the ALIS EHR system, reporting condition changes promptly, and participating in required training. They also support compliance with federal, state, and local regulations and perform basic housekeeping or laundry as needed. • Provide personal care (bathing, dressing, toileting, feeding) per care plan • Escort residents to meals and activities • Ensure residents are clean, dressed, groomed, safe, and fed during shifts • Document services in ALIS EHR and maintain secure electronic devices • Monitor and immediately report any change in resident condition • Attend all scheduled in‑service trainings • Follow federal, state, local regulations and community policies • Perform basic housekeeping and laundry as needed • Obtain and record resident weights and vitals when requested by RN

What You Bring

Applicants must have a high school diploma or GED, at least one year of resident care experience, and be certified as a Nursing Assistant or Home Health Aide. This non‑exempt, hourly position reports to the Health & Wellness Director and is available in full‑time, part‑time, and per‑diem shifts (3 pm‑11 pm and/or 11 pm‑7 am) with a wage of $21‑$23 per hour.
